Repeat Prescriptions
Repeat prescriptions can be ordered in person at reception, by post, fax, or email. Please do not request
items which have not been agreed as a repeat prescription. Please remember to order them in good time, because once the surgery
is closed there is no service available to provide repeat prescriptions. Prescriptions will be ready the next working
day after 1 p.m.

Sickness Certificates
The NHS does not provide sick notes for illnesses of less than seven days. If you need a sick note and you
have been ill for less than 7 days you should complete your own note called a SC2. This is available from benefits offices,
or your employer or can be dowloaded on the link here.
If your employer insists on a sick note when you are ill for less than 7 days then this would be a private
service, for which a charge will be made.

Practice Nurse
The Practice Nurse, Kath Kerry is normally available every day of the week, by appointment. She is able
to offer a wide range of services, including holiday vaccinations, new patient medicals, blood pressure checks, heart disease
screening, diabetes and asthma checks.
Cervical Screening Tests
The cervical screening clinic runs on Friday afternoons. If your appointment is not convenient, please ask
for it to be rearranged. The tests are designed to detect changes in the cervix, and prevent cancer of the cervix. For more

Telephone Advice
If you need advice or to speak to one of the doctors then please phone 01204 521574 between 11:00 and 11:30
a.m. It is helpful if you could give a brief outline of your problem to the receptionist as this makes for a more efficient
use of the doctor's time.

Test Results
If the doctor has arranged any blood tests please always phone the reception for the result one week
later. For X Ray results please phone after 4 weeks.
Childrens Immunisations
Appointments for childrens immunisations are sent out automatically, for the clinic which is run by Dr Faulkner on Friday
afternoon. If the appointment is not convenient, we can easily rearrange it for another time. If you have any concerns or
questions about immunisation please ask for an appointment for Dr Faulkner or click the link below for up to date information
from the Department of Health
Ante-Natal clinic
Tuesday afternoons 1:30 - 3:30 pm. The clinic is run by the midwife, please see reception if you need an appointment.
If you are pregnant, please make sure you are taking folic acid for the first three months, and remember that smoking is even
more harmful for your baby than it is for you. For advice on keeping healthy when you are pregnant click here
Stopping Smoking
Stopping smoking is the single most important thing that you can do to improve your health. We can support you with stopping
smoking literature, referral to NHS stop smoking sessions, and we prescribe nicotine replacement and other stop smoking drugs
